PATIENT GUIDE & PROCEDURES
This guide outlines the step-by-step process for students to access medical
treatments and healthcare services at UNIMAS Health Centre.
All services at the University Health Centre (PKU) are available FOR REGISTERED STUDENTS ONLY.
Please bring one of the documents when registering:
- Pre-University Students
- Student Matric Card, MyKad OR Offer Letter (New Students)
- Undergraduate Students
- Student Matric Card, MyKad OR Offer Letter (New Students)
- Postgraduate Students
- Student Matric Card, MyKad OR Offer Letter (New Students)
- Short-Program Students
- Student Matric Card, MyKad OR Passport
Note: Please ensure your student registration status is active to be eligible for services.
For new UNIMAS students, medical check-ups are generally conducted through the UNIMAS Health Centre (PKU).
Below is a brief guide to the medical check-up process and the important information you need to know:
Medical Check-Up Documents & Forms
1. Official Health Form: The Medical Examination and Health Declaration Form can be downloaded via the UNIMAS New Student Registration Portal, official UNIMAS Health Centre (PKU) website or through the attachment provided in you offer email.
2. Form Sections: The form generally consists of two sections:
- A section to be completed by the student (Health Declaration).
- A section that must be completed, verified and signed by a registered medical practitioner/medical officer conducting the medical examination, whether at an external clinic or government health facility.
3. External Medical Examinations & Endorsement: If the medical check-up is completed outside of PKU, the student must still present the completed report to PKU for endorsement by a PKU Medical Officer.
Note: Student submitting and external report do not need to queue for the routine medical check-up procedures at PKU.
- Service Availability: Emergency cases are handled at PKU until 5:00 PM only.
- Cases Classified as Medical Emergencies:
- Accidents and/or severe injuries
- High fever (Above 39°C)
- Difficulty breathing or shortness of breath
- Loss of consciousness or fainting
- Severe bleeding
- Any other critical conditions that may endanger the patient or others
- Service Availability: Ambulance services are available until 5 pm only.
- Strictly for Emergencies: The ambulance is strictly reserved for medical emergencies. Please refer to the Medical Emergencies section above for examples of eligible conditions.
- Event Eligibility: A Medical Standby Team may be requested for university events/activities.
- Submission Timeline: The official request letter must be submitted to the clinic registration counter at least three (3) days before the event.
- No Backdated MC: Request for backdated Medical Certificates (MCs) will not be issued or entertained.
- In-Person Requests: Requests for endorsement of an external Medical Certificate (MC) must be made in person by the student concerned.
- Use of Representatives: A representative can only act on behalf of a student if the patient is a postpartum case or has a severe physical condition that prevents them from attending in person (e.g., a leg fracture).
- Eligibility: GL applications are available for local students with active status only.
- Form Submission: Complete the GL application form and submit it to the clinic registration counter.
- Medical Review: A copy of the relevant medical records or supporting medical document is required for review by the Medical Officer (MO).
- Collection: Once approved, the GL may be collected personally by the applicant or by an authorized representative.
- Form Submission: Complete the application form and submit it at the clinic registration counter.
- Only the following medical equipment is available for loan:
- Wheelchair
- Crutches
- Loan Period: Medical equipment may be borrowed for a period of up to seven (7) days only.
- Equipment Responsibility: Students are fully responsible for the care of the borrowed equipment. In the event of loss or severe damage, the student must replace or pay for the equipment.
- Form Availability: The First Aid Refill Form is available for download.
- Approval: The completed form must be signed and approved by the applicant's Head of Department or Program Coordinator before submission.
- Submission Timeline: The completed form must be submitted to the clinic registration counter at least three (3) days before the event or activity.
- First Aid Box Policy: UNIMAS Health Centre (PKU) does not provide physical first aid boxes or containers. PKU provides medications and supplies for refilling first aid boxes only. Please bring your own first aid box or container when submitting the request.
- Special Requests: If a required medication or item is not listed in the form, applicants are advised to consult the Medical Officer (MO) directly.
- Students may request transportation to the government hospitals, clinics or health facilities through the clinic registration counter.
- In-Person Booking: The booking request must be made in person by the student concerned. Representatives are not permitted, except in cases involving medical emergencies or physical conditions that prevent the student from attending in person.
